If you are new to our practice, you can complete our New Patient Registration online:

Or download and complete the Patient Registration Form (link below). Once the form is completed you can fax, email, post or bring it with you to your initial consultation.
If you wish to transfer your medical records from another doctor’s surgery, please complete the Records Transfer form (link below).
For existing patients to complete, in the event of a change of address, contact number etc.
For existing patients who wish to request their medical records from other health clinics i.e. hospital, radiological clinic (Perth Radiological Centre (PRC), SKG, InSight) etc.

South Pacific Underwater Medicine Society (SPUMS)
South Pacific Underwater Medicine Society (SPUMS) plays a pivotal role in diving safety in Australia as an NGO, advising on State and Federal legislation and is represented on several Australian Standards committees. Perhaps its most valuable contribution is the SPUMS recreational diving medical, advised for all divers undertaking initial dive training and then every five years for those 45 years or older. The SPUMS website provides a readily accessible list of doctors trained to perform recreational and commercial diving medicals.

The Divers Alert Network (DAN) is the diving industry’s largest association dedicated to scuba diving safety. Serving scuba divers for more than 30 years, DAN provides emergency assistance, medical information resources, educational opportunities and more.
